The activation of TGF-b could account for the observed myofibroblast transformation at week three, in accordance with earlier studies.11 26 27 In addition, the observed expression of ED-A fibronectin has been reported as critical for a-SMA expression.28 Myofibroblasts are recognized to result in wound contraction following, for instance, full thickness incisional wounds and radial keratotomy, major to changeswww.bjophthalmol.comIvarsen, Laurberg, M ler-Pedersenin corneal curvature.291 Thus, the present discovering of myofibroblasts in the LASIK flap edge might indicate an active wound contraction inside the region. Besides the ability to induce myofibroblast transformation, TGF-b1 and TGF-b2 are generally accepted as fibrogenetic growth variables.27 32 Also the significance of CTGF (a presumed downstream mediator of TGF-b)32 33 for the development of fibrosis has previously been recognised.34 35 In the present study, the stromal expression of TGF-b1, TGF-b2, and CTGF indicate that all of these development aspects are involved inside the fibrotic wound repair. The strict localisation of fibrosis peripheral to the LASIK flap edge, contrary for the minimal fibrosis under the whole flap, suggests that the LASIK interface within the rabbit cornea by no means heals. These findings in rabbits deliver a structural explanation for the clinically observed lack of post-LASIK wound healing that makes it possible for the corneal flap to become separated from its stromal bed for an indeterminate time immediately after surgery.four five Given that the rabbit cornea otherwise is recognized to heal aggressively (following, for instance, photorefractive keratectomy or possibly a penetrating wound),368 LASIK gives an exciting model for studying the fundamental regulation of corneal wound repair plus the improvement of fibrosis.
